Location: Atlanta Title: Case Manager Program: Foster Care
Summary:
The Case Manager is primarily responsible for the development and support of the service plans for individuals receiving services through specialized and therapeutic foster care. The manager must serve as an advocate to ensure proper placement and services and will make regular and unscheduled visits to the home, school, training site or job of the individuals served. The manager will help provide emergency coverage and crisis intervention support to foster care placements. Maintains accurate, current and complete service records according to established timelines. Attends court hearings, panel reviews, staffing, supervised visits and other meetings. Teaches problem solving techniques and education trainings. Evaluates the caregivers and their homes for safety problems and contract compliance. Conforms with and abides by all regulations, policies, work procedures and instructions. Works in collaboration with children served, their families, providers and community partners.
Qualifications:
Bachelor's Degree in social work or another social science field such as sociology or psychology from an accredited school. Two years previous experience such as direct casework or supervision of individuals receiving services with special needs. Demonstrated skills in assessment, negotiation and problem solving. Ability to function independently and have flexibility, personal integrity and ability to work effectively with children, co-workers and support agencies. Medically fragile and Therapeutic Foster Care experience is preferred. Demonstration of skills to include the ability to make difficult decision, strong leadership skills and excellent verbal and written communication. Computer skills with proficiency in Microsoft Word, Excel, Power Point and Access is required. Must own a personal multi-passenger vehicle, hold a valid Georgia's license, have a good driving record, appropriate insurance and be willing to drive clients as needed.
